form to our company as follows:

October 1st., mail or fax to us the completed reservation
form below, with a deposit check on the amount of $500.00
per booth required. Credit cards are also acceptable. |

After
October 1st., mail or fax to us the completed reservation
form below with a check on the amount of $1650.00 per booth
required. Credit cards are also acceptable |

If contracted and paid before October 1st., $1,500 per booth required. |
